Conference summary

Pain management in dentistry stands as one of the most critical cornerstones of successful clinical practice. Effective pain control not only alleviates patient distress but also mitigates systemic physiological stress.
 
Selecting an appropriate analgesic regimen is a clinical challenge and it demands a highly individualized approach. The clinician must balance therapeutic efficacy against the potential for adverse drug events, a task that becomes complicated when managing complex patients,  who present with preexisting systemic pathologies or complex polypharmacy regimens.
 
From a pharmacological perspective, clinicians primarily rely on three distinct classes of analgesics, each characterized by different mechanisms of action and specific toxicity profiles.
 
Paracetamol remains a first-line agent for mild-to-moderate pain, acting primarily through central nervous system pathways to inhibit prostaglandin synthesis; however, its administration requires strict adherence to daily dose thresholds to prevent hepatotoxicity.
 
Nonsteroidal anti-inflammatory drugs (NSAIDs), are highly effective in the management of dental pain because they inhibit cyclooxygenase enzymes, thereby reducing the synthesis of prostaglandins responsible for the peripheral sensitization of nociceptors.
 
Nonetheless, NSAIDs carry pronounced risks of gastrointestinal ulceration, acute renal impairment, and adverse cardiovascular events. In cases of severe postoperative pain, opioid analgesics may be indicated, although their use may be associated with adverse effects.
 
In addition to these main drug classes, adjuvant medications, such as corticosteroids and anticonvulsants, are playing an increasingly important role in pain management strategies.
 
The selection and dose adjustment of these medications depend on a careful assessment of both the characteristics of the pain (including its intensity, duration, and underlying etiology) and the patient's underlying medical conditions. Complex patients comprise specific vulnerable populations in whom standard empirical dosing may lead to serious complications.
 
In conclusion, achieving optimal analgesia in complex patients requires a comprehensive clinical assessment.
 
By identifying high-risk individuals, recognizing potential drug interactions, and appropriately tailoring therapeutic regimens according to evidence-based multidisciplinary guidelines, clinicians can achieve effective pain control while minimizing the risk of systemic complications.
