Conference summary

We all know that dental avulsion is one of the most serious problems in dentistry and that it requires an immediate response, which is why we are perfectly familiar with the protocol for each of its phases (extraoral time, conservation medium, etc). But since the prognosis depends on correct management of the pulp and periodontal ligament, let’s focus on all those extreme situations that compromise the prognosis and make us doubt the steps to be taken to prevent tooth loss.

After replantation, revascularization is only possible in immature teeth whose apical diameter is greater than 1.5 mm; In these cases, within 15 days after replantation, the coronal pulp is replaced by proliferative mesenchymal cells and capillaries. Regarding the periodontal ligament, in case of repair, after hemostasis there is a period of inflammation, cellular and fibroblast proliferation, collagen synthesis, remodeling and, finally, healing.

If this reparative process changes, a non-physiological root resorption process can be observed. This implies an eventual and/or permanent loss of the mineralized root tissue (dentin and cementum) due to a pathological odontoclastic action.
In avulsion, if the predentin is damaged in any area of the root, the tooth can present an inflammatory resorption (in 30% of cases) or a resorption by replacement or ankylosis (in 60% of cases).

Therefore, we will discuss the possible complications (of the periodontal ligament and of the pulp) that may appear during the follow-up of the reimplanted tooth and the different treatment options that can be applied to each of these complications.
