Conference summary

Endo-Perio Lesions (EPLs) establish a pathological communication between endodontic and periodontal tissues, presenting as either acute or chronic conditions. It is recommended to classify EPLs based on signs and symptoms that directly impact prognosis and treatment, including the presence or absence of fractures and perforations, the presence or absence of periodontitis, and the extent of periodontal destruction around the affected teeth.

Under physiological conditions, a balance exists between the periodontal supporting tissues and the pulp/root canal complex. If the pulp or periodontium experiences injury, microorganisms and inflammatory products affecting one structure may also impact the other. Often, these pathological communications are contained after effective periodontal or root canal treatment. For instance, even if a certain degree of cross-contamination with the periodontium occurs when the root canal is infected, it usually disappears after appropriate root canal treatment. However, when severe damage occurs to both the pulp/root canal complex and the periodontium in the same tooth, accompanied by a deep periodontal pocket and altered sensitivity, it is referred to as an endo-perio lesion (EPL).

The therapeutic approach of EPLs involves an interdisciplinary management aiming to address both the endodontic and periodontal components of the lesion. Accurate identification of the type of EPL based on signs and symptoms allows for appropriate treatment planning, which may include endodontic therapy, periodontal therapy, periodontal regeneration, and effective infection control. Collaboration between periodontists and endodontists is crucial to achieve favorable outcomes in the treatment of EPLs.

In conclusion, the treatment of Endo-Perio Lesions in periodontology requires an individualized interdisciplinary approach. Understanding the clinical and biological aspects of EPLs, along with the application of appropriate techniques and therapies, contributes to treatment success. Through collaboration between specialists, we can restore oral health and preserve the great majority of the teeth affected by EPLs.
